Objective: Although hypothermic circulatory arrest (HCA) and selective cerebral perfusion (SCP) are widely used for cerebral protection during aortic arch surgery, these strategies offer no protection for mesenteric ischemia. This study explored mesenteric hemodynamics, metabolism, oxidative stress and splanchnic inflammatory response during isolated SCP respectively combined cerebral and lower body perfusion (CLBP) in pigs.

Methods: 14 pigs (35±3kg) were cooled on CPB to 27°C. After 10min of HCA they were randomized to 60min of isolated SCP (n=7) respectively CLBP (n=7) at pump rates of 10ml/kg/min each. Microspheres were injected at baseline, 5 and 60min of SCP/CLBP, respectively 5 and 60min off CPB, to calculate mesentric blood flow (BF) and metabolic rate of oxygen (MRO2). Oxy-DNA expression was measured using flow cytometry analysis of portal blood at four time points. Mesenteric TNFα and IL6 were analyzed using real-time-PCR and immunhistology.

Results: Baseline blood flow was 61±31ml/min/100g in the jejunum and 78±43ml/min/100g in the colon. Whereas SCP provided a residual mesenteric BF of 5%, CLBP offered 47% of the baseline jejunal (34±10ml/min/100g) and 68% of the colonal BF (52±34ml/min/100g) (p=0.001). Oxy-DNA expression increased after HCA reaching 141% of baseline (SCP) and 108% (CLBP) at 60 minSCP/CLBP. 60min off CPB it reached 120% in both groups. The TNFα-mRNA-expression was higher in the SCP-group: 1.05±0.02 vs. 0.97±0.04 (p=0.04).

Conclusion: Low-flow CLBP provides a diminished but considerable mesenteric BF. In the early post-HCA period, CLBP leads to lower oxidative stress and inflammatory response reaction in the portal blood respectively the splanchnic tissue than isolated SCP.
